This form caters for two types of registrants:
1) Alumni who have registered onto the Alumni Database previously: This form will allow you to pre-register to have live access to your record on the Alumni Database in order to update your contact details and employer information whenever they change. Key fields on this form (First and Last Name, Date of Birth and Email address) need to match with the information we have in our records on the Database. Note that if the information you enter on the form below for these key fields does not match with what we have on the Database, you will be prompted to provide more information on a second form that will allow us to research your record and get back to you via email with the way forward. Once you complete this form, you will receive an email with a temporary password which will allow you to log into the Database from the Stay Connect page. Once you log in, you will be prompted to change your temporary password to a password of your choice.
For Alumni who already have a Database log in password, sign in is only possible through the Stay Connect page.
2) New registrants: If you are a new registrant, this form will allow you to provide the information we need to research and verify your record. Once you fill out the form below, you will be prompted for more information on a second form. Following this standard process, we will communicate with you via email with the way forward.
